RESOLUTION INTRODUCED BY HEALTH AND HUMAN SERVICES COMMITTEE TO AUTHORIZE COUNTY MANAGER TO ENTER INTO AGREEMENT BETWEEN DEPARTMENT OF PUBLIC HEALTH AND HUDSON VALLEY HOME REHAB, LLC.
WHEREAS, the County of Sullivan, through the Department of Public Health are executing an agreement for services for Physical Therapy; and
WHEREAS, the Department of Public Health wishes to award a R 23-25 to Hudson Valley Home Rehab, LLC for the period of September 1, 2023 through August 31, 2024. This agreement may be extended, upon mutual agreement, for three (3) additional years on a yearly basis at an amount not to exceed $300,000.00 a year, with the Physical Therapist visit fee of $95.00 per visit and a Physical Therapist Assistant at $85.00 per visit; and
NOW, THEREFORE, BE IT RESOLVED, the Sullivan County Legislature does hereby authorize the County Manager to execute an agreement with Hudson Valley Home Rehab, LLC for the period of September 1, 2023 through August 31, 2024. This agreement may be extended, upon mutual agreement, for three (3) additional years on a yearly basis; and
BE IT FURTHER RESOLVED, the maximum of the agreement is not to exceed $300,000.00 a year, with the Physical Therapist visit fee of $95.00 per visit and a Physical Therapist Assistant at $85.00 per visit; and
