RESOLUTION INTRODUCED BY EXECUTIVE COMMITTEE TO ENTER INTO AN AGREEMENT BETWEEN THE COUNTY OF SULLIVAN AND SULLIVAN 180
WHEREAS, the County of Sullivan (“County”) and Sullivan 180 desire to entered into an Agreement whereby Sullivan 180 agrees to provide a pilot program to directly stipend community health champions as grassroots community health workers for the purposes of teaching evidence based prevention programs (English and Spanish) that align with the County’s Health Improvement Plan and/or mission of Sullivan 180 out in the community; and
WHEREAS, the County agrees to provide Sullivan 180 with an amount not to exceed ten thousand dollars ($10,000.00).
NOW, THEREFORE, BE IT RESOLVED, that the County of Sullivan Legislature hereby authorizes the County Manager to execute a contract with Sullivan 180 in an amount not to exceed $10,000.00 for the purpose of education in a form to be approved by the County Attorney.
