RESOLUTION INTRODUCED BY EXECUTIVE COMMITTEE TO TERMINATE A CONTRACT BETWEEN THE COUNTY OF SULLIVAN AND SULLIVAN 180 DATED NOVEMBER 30, 2022
WHEREAS, the County of Sullivan (“County”) and Sullivan 180 entered into an Agreement dated November 30, 2022 for Sullivan 180 to provide leadership and planning for a coordinated network of health care workers in Sullivan County to address the issue of chronic disease and to reduce unnecessary repeat hospitalization and emergency department visits by residents disproportionately affected by social and economic factors pursuant to resolution No. 409-22 adopted by the Sullivan County Legislature on September 20, 2022; and
WHEREAS, the County agreed to provide Sullivan 180 with an amount not to exceed fifty thousand dollars ($50,000.00); and
WHEREAS, Sullivan 180, having expended approximately forty thousand dollars ($40,000.00) now desires to terminate the contract and enter into a new agreement; and
WHEREAS, the County is in agreement with termination of the November 30, 2022 contract.
NOW, THEREFORE, BE IT RESOLVED, that the contract between the County of Sullivan and Sullivan 180 dated November 30, 2022 is hereby terminated.
