RESOLUTION NTRODUCED BY THE HEALTH AND HUMAN SERVICES COMMITTEE TO AUTHORIZE PUBLIC HEATH SERVICES TO APPLY FOR AND IF AWARDED, ACCEPT FUNDING FOR THE HEALTH RESEARCH, INC. RURAL HEALTH NETWORK TECHNICAL ASSISTANCE GRANT


WHEREAS, the Sullivan County Public Health Department is required to provide a Community Health Needs Assessment (CHNA) in 2022, and to accomplish this, technical assistance is needed to help collect, collate and assimilate the information obtained by focus groups, surveys and other information gathered by Public Health over the next few months into a comprehensive report; and

WHEREAS, Health Research, Inc. (HRI) and the New York State Department of Health (NYSDOH) have notified Rural Health Networks that they are eligible to apply for a Technical Assistance Grant of $10,000 for Rural Health projects that are directly related to Rural Health goals- requirements which the CHNA meets; and

WHEREAS, the term of this grant is from April 1, 2022 to June 30, 2022, with projects to be completed by June 30, 2022, in the amount of $10,000, dependent upon HRI/ NYSDOH funding availability; and

WHEREAS, if awarded, the funding from this grant will be used to hire a subcontractor to assimilate all survey data received from Siena College as part of our CHNA process, incorporate additional information from focus groups, interviews and updated health statistics, and create a comprehensive report that Public Health will determine that meet state requirements for the Community Health Assess...
