RESOLUTION INTRODUCED BY HEALTH AND HUMAN SERVICES TO DO A MODIFICATION AGREEEMNT WITH MID-HUDSON PSYCHIATRIC CONSULTANTS TO INCREASE THE HOURLY RATE.
WHEREAS, the County of Sullivan entered into an original agreement with Mid-Hudson Psychiatric Consultants dated April 1, 2022, in accordance with Resolution No. 171-21, adopted by
the Sullivan County Legislature on April 15, 2021, as per RFP #R-21-06; and
WHEREAS, Mid-Hudson Psychiatric Consultants has surpassed the caseloads due to Dr.
Nambi Salgunan being out on medical; and
WHEREAS, the hourly rate of services shall be increased from $200.00 to $205.00,
per hour; and
WHEREAS, this modification agreement shall take effect on May 1, 2022; and
WHEREAS, the Sullivan County Department of Community Services has
approved said increase and recommends the agreement be modified.
NOW, THEREFORE, BE IT RESOLVED, the County Manager be and herby is authorized
to modify the agreement with Mid-Hudson Psychiatric Consults at $205.00, per hour, to take
effect on May 1, 2022 in accordance with RFP #R-21-06, said contract to be in
such form as the County Attorney shall approve.
