RESOLUTION INTRODUCED BY THE HEALTH & HUMAN SERVICES COMMITTEE TO EXTEND THE AGREEMENT WITH MID-HUDSON PSYCHIATRIC CONSULTANTS FOR A PERIOD OF SIX (6) MONTHS FOR PSYCHIATRIC SERVICES AT COMMUNITY SERVICES

                     WHEREAS, the Sullivan County Legislature (“Legislature”) authorized a contract with Mid-Hudson Psychiatric Consultants, Inc., Quazi Al-Tariq, MD, for psychiatric services at Community Services pursuant to Resolution No. 171-21; and

                     WHEREAS, the Legislature modified said agreement pursuant to Resolution No. 145-22; and

                     WHEREAS, there is a need to extend said agreement for six (6) months while the County works to issue another Request for Proposals. 

                     NOW THERFORE BE IT RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager to extend the agreement with Mid-Hudson Psychiatric Consulting, Inc. for a period of six (6) months; and

                     BE IT FURTHER RESOLVED, said agreement to be in a form approved by the Sullivan County Attorney.
