WHEREAS, the Sullivan County Department of Public Health is responsible for carrying out public health duties and program requirements established by federal and New York State laws, regulations, contracts, grants, cooperative agreements, and program guidance; and
WHEREAS, the performance of these responsibilities requires the Department to access and use electronic platforms maintained or approved by the New York State Department of Health, the Centers for Disease Control and Prevention, and other authorized state and federal agencies; and
WHEREAS, these systems are used for functions including, but not limited to, disease surveillance, case investigation and follow-up, submission of required reports, management of state and federal program deliverables, secure exchange of protected health information, and communication with state and federal public health partners; and
WHEREAS, access to these platforms may require the execution of user agreements, access agreements, confidentiality attestations, data-use agreements, security acknowledgments, participation agreements, and other similar documents as a condition of access or continued participation; and
WHEREAS, timely execution of these routine agreements is necessary to avoid delays in patient follow-up, disease-control activities, required reporting, and the Department's ability to meet mandated public health responsibilities; and
