RESOLUTION INTRODUCED BY EXECUTIVE COMMITTEE TO MODIFY RESOLUTION 415-24

WHEREAS, Resolution #415-24 was approved by the Sullivan County Legislature on 8/15/2024, authorizing execution of an agreement with CarePort Health, LLC an affiliate of WellSky Corporation, with an office address of 11300 Switzer Road Overland Park, KS 66210, Wellsville, New York 14895, to provide Public Health Services access to the online, cloud-based "CarePort Referral Management" Solution for the period October 9, 2024 through October 8, 2025, with the option to extend on a yearly basis for four (4) additional years; and; and
WHEREAS, the funding and contract period language in the Original Resolution is incorrect, and needs to be amended for said agreement, and
WHEREAS, the agreement shall continue in effect for successive continual one (1) year periods at a cost not to exceed a 6% annual increase over the prior year, provided that such renewal is approved by the Director of Information Technology Services and authorized by a modification agreement executed by both parties.
NOW, THEREFORE, BE IT RESOLVED that Resolution #415-24 is hereby amended to include that the agreement shall continue in effect for successive continual one (1) year periods at a cost not to exceed a 6% annual increase over the prior year, provided that such renewal is approved by the Director of Information Technology Services and authorized by a ...
