RESOLUTION INTRODUCED BY THE HEALTH AND HUMAN SERVICES COMMITTEE TO RENEW THE EXISTING CONTRACTS FOR PERSONAL CARE AND HOME HEALTH AIDESend

WHEREAS, Resolution # 175-21 authorized the County Manager to execute contracts with six Home Health Care Agencies for Home Health Aides and Personal Care Aide services from July 1, 2021 through June 30, 2022, with an option to extend on a yearly basis, for four (4) additional years under the same terms and conditions; and

WHEREAS, the Sullivan County Department of Public Health continues to recommend said contracts; and
NOW, THEREFORE, BE IT RESOLVED, that the County Manager is hereby authorized to execute the renewal of the above contracts from 7/1/2024 through June 30, 2025, under the same terms and conditions, with the above agencies at the price per hour listed above. Said contracts to be in such form as the County Attorney shall approve in accordance with RFP R-21-09.-21-09
