RESOLUTION INTRODUCED BY EXECUTIVE COMMITTEE TO AMEND RESOLUTION NO. 165-26 AND AUTHORIZE AWARD AND EXECUTION OF A CONTRACT WITH ALWAYS COMPASSIONATE HOME CARE, INC. FOR PERSONAL CARE AND HOME HEALTH AIDE SERVICES
WHEREAS, Resolution No. 165-26 authorized the County Manager to execute agreements with various Contractors, in accordance with Request for Proposal #R-26-13 for Personal Care and Home Health Aide services for Sullivan County; and
WHEREAS, the resolution needs to be modified to include an additional Contractor; and
WHEREAS, the County now desires to add Always Compassionate Home Care, Inc., 265 Broadhollow Road, Suite 200, Melville, New York 11747, as an additional contractor to provide Personal Care and Home Health Aide services at the contractor rate of $40.00 per hour, with a guaranteed minimum rate of $24.00 per hour to the aide, for the contract period of July 1, 2026 through June 30, 2027, with an option to extend on a yearly basis for four additional years under the same terms and conditions; and
WHEREAS, the Sullivan County Department of Public Health Services and the Sullivan County Office for the Aging recommend the award of an agreement to Always Compassionate Home Care, Inc.
