A RESOLUTION INTRODUCTED BY HEALTH AND HUMAN SERVICES TO AMEND RESOLUTION # 187-25
WHEREAS, the Sullivan County Legislature adopted Resolution #187-25 authorizing Sullivan County Department of Public Health to enter into an agreement with New York State Department of Health (NYSDOH) for the Rabies Prevention Program pending contract finalization; and
WHEREAS, the NYSDOH has awarded the Sullivan County Department of Health a two-year extension of their contract C36563GG starting April 1, 2025 through March 31, 2027 in the amount of $19,047.00 annually. The amended five-year total contract value is $95,235.00; and
NOW, THEREFORE, BE IT RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, Chairman of the County Legislature, and / or their authorized representative (as required by the funding source) to execute any and all necessary documents to submit the extended fiscal year 22 NYSDOH Rabies Prevention Program Contract from April 1 2022 through March 31, 2027 contract for funding; and
BE IT FURTHER RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, and / or Chairman of the County Legislature (as required by the funding source) to accept the award, and enter in to the extended fiscal year 22 NYSDOH Rabies Prevention Program Contract from April 1 2022 through March 31, 2027 to administer the funding secured, in such form as the County Attorney shall approve; and
BE IT FURTHER RESOLVED, that should the Rabies Prevent Program funding be terminated, the County shall not be obligated to continue any action undertaken by this funding.
