RESOLUTION INTRODUCED BY THE --EXECUTIVE COMMITTEE TO APPLY FOR AND ACCEPT FUNDING FROM THE TUBERCULOSIS (TB) ELIMINATION ALLIANCE MINI-GRANTS PROGRAM
WHEREAS, the Sullivan County Department of Public Health has the opportunity to apply for and receive funding through the Tuberculosis (TB) Elimination Alliance Mini-Grants Program, supported by the Centers for Disease Control and Prevention (CDC), to strengthen tuberculosis prevention, education, outreach, and quality improvement efforts; and
WHEREAS, the grant provides funding in an amount up to $15,000 for the grant period October 1, 2026 through May 31, 2027; and
WHEREAS, awardees will enter into contract with the Tuberculosis Elimination Alliance Mini-Grants Program in August of 2026 and receive an advance of funding prior to the start of the Grant Period in September 2026; and
WHEREAS, the proposed project aligns with the Sullivan County Community Health Improvement Plan and the Department's communicable disease prevention priorities by supporting tuberculosis education, provider engagement, outreach to populations at increased risk, and quality improvement activities; and
WHEREAS, no County matching funds are required to accept this grant award; and
WHEREAS, all grant awards must be approved by the Sullivan County Legislature prior to acceptance and execution of the necessary grant documents.
NOW, THEREFORE, BE IT RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, Chairman of the County Legislature, and/or their authorized representative (as required by the funding source) to execute any and all necessary documents to submit the Tuberculosis (TB) Elimination Alliance Mini-Grants Program; and
BE IT FURTHER RESOLVED, that the Sullivan County Manager hereby designates the Director of Public Health as the authorized representative to execute any and all documents to apply for the Tuberculosis (TB) Elimination Alliance Mini-Grants Program in an amount up to $15,000 for the period October 1, 2026 through May 31, 2027; and
BE IT FURTHER RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, and/or Chairman of the County Legislature to accept the award, and execute any and all documents required to administer the funding secured, in such form as the County Attorney shall approve; and
BE IT FURTHER RESOLVED, that should the Tuberculosis (TB) Elimination Alliance Mini-Grants Program funding be reduced or terminated, the County shall not be obligated to continue any activities undertaken solely through the use of these grant funds.
