RESOLUTION INTRODUCED BY THE EXECUTIVE COMMITTEE TO APPLY FOR AND ENTER INTO AN AGREEMENT TO ACCEPT FUNDING FROM NEW YORK STATE ASSOCIATION OF COUNTY HEALTH OFFICIALS (NYSACHO) FOR THE IMMUNIZATION ACTION PLAN PROJECT

 

 

WHEREAS, New York State Association of County Health Officials (NYSACHO), in partnership with the New York State Department of Health (NYSDOH), has released a Request for Applications for Immunization Action Plan (IAP) Mini Grants to support local health departments in improving vaccination coverage, addressing disparities, and reducing barriers to immunization; and

 

WHEREAS, this grant provides funding of up to $5,000 to support local, immunization initiatives that enhance outreach, education, and access to vaccines;

 

WHEREAS, the Sullivan County Department of Public Health operates a Diagnostic and Treatment (D&T) Clinic that provides Vaccines for Children (VFC) and other immunization services and seeks to increase vaccination rates countywide, particularly among underserved and high-risk populations; and

 

WHEREAS, the proposed project will align with NYSDOH and NYSACHO priorities to improve vaccine confidence, reduce access barriers, and increase immunization uptake through community-based strategies; and

 

WHEREAS, applications for this funding opportunity are due by May 1, 2026, and selected projects will be implemented beginning June 2026;

 

NOW, THEREFORE, BE IT RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, Chairman of the County Legislature, and/or their authorized representative (as required by the funding source) to execute any and all necessary documents to NYSACHO for the Immunization Action Plan Mini Grant Program in an amount not to exceed $5,000; and

 

BE IT FURTHER RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, and / or Chairman of the County Legislature (as required by the funding source) to accept the award, and enter into an award agreement or contract to administer the funding secured, in such form as the County Attorney shall approve; and

 

BE IT FURTHER RESOLVED, that this grant will supplement existing programs and will not require additional County matching funds or create a long-term financial obligation to the County; and

 

BE IT FURTHER RESOLVED, that should the NYSACHO funding be terminated, the County shall not be obligated to continue any action undertaken by the use of this funding.
