RESOLUTION INTRODUCED BY THE --EXECUTIVE COMMITTEE TO APPLY FOR AND ACCEPT FUNDING FROM THE FIDELIS CARE HERE FOR YOUR HEALTH MATERNAL HEALTH GRANT PROGRAM
WHEREAS, the Sullivan County Department of Public Health has the opportunity to apply for and receive funding through the Fidelis 2026 Here for Your Health Maternal Health Grant Program to support healthcare providers and community-based organizations throughout New York State in implementing innovative strategies to improve maternal health outcomes; and
WHEREAS, Fidelis Care has made available up to $20,000 in grant funding through this initiative, with applications due September 21, 2026;
WHEREAS, the grant program is intended to support initiatives that improve postpartum visit adherence, expand maternal depression screening, reduce barriers to care, and connect pregnant and postpartum individuals with appropriate healthcare and supportive services, particularly within underserved communities; and
WHEREAS, Sullivan County continues to experience barriers to maternal healthcare access associated with its rural geography, limited healthcare provider capacity, transportation challenges, socioeconomic disparities, and other factors that may affect timely access to prenatal, postpartum, behavioral health, and supportive services; and
WHEREAS, funding through the Fidelis Care Here for Your Health Maternal Health Grant Program would provide an opportunity to enhance existing maternal and child health initiatives, strengthen postpartum outreach and follow-up, promote maternal depression screening, improve connections to clinical and behavioral health services, and address barriers that contribute to gaps in postpartum care; and
NOW, THEREFORE, BE IT RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, Chairman of the County Legislature, and/or their authorized representative (as required by the funding source) to execute any and all necessary documents to submit the 2026 Here for Your Health Maternal Health Grant Program; and
BE IT FURTHER RESOLVED, that the Sullivan County Manager hereby designates the Director of Public Health as the authorized representative to execute any and all documents to apply for the 2026 Here for Your Health Maternal Health Grant Program; and
BE IT FURTHER RESOLVED, that the Sullivan County Legislature hereby authorizes the County Manager, and/or Chairman of the County Legislature to accept the award, and execute any and all documents required to administer the funding secured, in such form as the County Attorney shall approve; and
BE IT FURTHER RESOLVED, that should funding be awarded, the Sullivan County budget shall be modified accordingly to reflect the receipt and expenditure of such grant funds.
